Bleeding Gums and Gum Disease: What It Means and What to Do

If the sink turns pink when you brush, that is not something to shrug off and it is not something to panic about either. It is the earliest warning sign of gum disease, and at that stage it is almost always reversible.

The short answer

Healthy gums do not bleed. Bleeding when you brush or floss means the gums are inflamed, which is gingivitis, and it is caused by plaque sitting undisturbed along the gumline. Caught at this stage it can be reversed completely with better cleaning and a hygiene appointment. Left alone for years it can progress to periodontitis, where the bone supporting the teeth is lost, and that part cannot be undone. Gum disease, not decay, is the leading cause of tooth loss in British adults.

Why gums bleed

  • Plaque along the gumline. The overwhelming majority of cases. Plaque is a soft bacterial film that reforms within hours, and where it sits undisturbed for a day or two the gum responds by becoming inflamed. Brushing removes it. Missing the same spot every night is why bleeding is often localised to one area.
  • Not cleaning between the teeth. A toothbrush cannot reach the surfaces between teeth, which is exactly where gum disease starts. This is the single most common gap in an otherwise good routine.
  • Brushing too little, not too hard. People often stop brushing an area because it bleeds, which makes it worse. Gums that bleed need cleaning more carefully, not less.
  • Smoking. A major risk factor, and it masks the signs. Smokers’ gums bleed less because the blood supply is reduced, so the disease progresses more quietly and more quickly.
  • Hormones and health conditions. Pregnancy, puberty and the menopause all make gums more reactive. Diabetes and gum disease worsen each other in both directions. Some medications reduce saliva or cause gum overgrowth.
  • Hardened tartar. Once plaque calcifies it cannot be brushed off and it holds new plaque against the gum. That needs removing professionally.

When it is more than gingivitis

Bleeding on its own usually means the problem is still at the reversible stage. Get seen sooner rather than later if you also have gums that have visibly receded, teeth that feel loose or have started to shift, a persistent bad taste or bad breath, or pus around a tooth. Those suggest the supporting bone is involved.

Also worth a prompt appointment: a single area that bleeds heavily when the rest of your mouth is fine, or a swelling that is growing. And if you are pregnant, gum care matters more rather than less, so do not put visits off.

What actually fixes it

  • Brush twice a day for two minutes with a fluoride toothpaste, angling the bristles into the gumline rather than scrubbing across it. An electric brush with a pressure sensor takes the guesswork out.
  • Clean between every tooth once a day. Interdental brushes suit most people better than floss and are considerably easier to do properly. Your hygienist will size them for you.
  • Expect bleeding to increase for the first few days once you start cleaning between the teeth, then settle. That is the inflammation resolving, not damage.
  • Do not rely on mouthwash. It reduces bacteria on the surface but does not remove plaque, and it cannot reach where the problem is.
  • Stopping smoking does more for your gums than anything else on this list.

What it costs

Frequently asked questions

Is it normal for gums to bleed when you floss?

No. It is common, which is not the same thing. If gums bleed when you clean between them it means they are inflamed. Keep cleaning gently and consistently and the bleeding should stop within one to two weeks. If it does not, get it looked at.

Can gum disease be cured?

Gingivitis, yes, completely. Periodontitis, no. Once bone has been lost around a tooth it does not grow back, but the disease can be stabilised and stopped from progressing, and teeth can be kept for decades in that state. That difference is the whole reason for acting on bleeding early.

Will a scale and polish stop my gums bleeding?

It removes the tartar you cannot shift yourself, which is a necessary part of it, but the appointment alone will not fix the problem if the daily routine does not change. Roughly speaking, we remove what has hardened and you control what happens between visits.

Does gum disease affect the rest of my health?

There is a well established link with diabetes, where each condition makes the other harder to control, and associations with heart disease and adverse pregnancy outcomes are widely reported. The mechanisms are still debated, so treat it as a good reason to look after your gums rather than a cause for alarm.
