A hygiene visit from start to finish

If you have never seen a hygienist, or it has been a while, it helps to know the shape of the appointment before you sit down. Wondering whether it hurts is normal, so let us answer plainly. Most patients find scaling comfortable, more a scratching sensation than pain. Where gums are inflamed or teeth sensitive it can be sharper, and there are two honest responses to that: tell your hygienist, who can adjust technique and pace, and remember that the discomfort fades as your gums heal between visits. The second or third appointment is nearly always easier than the first, because healthier gums simply feel less.

The 20-minute visit at £60 suits patients with healthy gums, modest tartar and a solid home routine: maintenance, in other words, done regularly to stay ahead. The 30-minute visit at £89 exists for mouths that need genuine time: heavier tartar, staining that wants proper attention, gums that bleed, a first hygiene visit after years away, or simply more teeth with more history around them.

If you are unsure, the honest guidance is this: book longer for a first visit, then let your hygienist tell you what maintenance should look like from there. Squeezing a neglected mouth into twenty minutes helps nobody; the work is either rushed or unfinished. Once things are stable, most patients settle happily into the shorter visit, and being told you can step down to it is a small badge of honour.

Frequency is the other half of the equation. Six-monthly suits most people, but it is a starting point rather than a rule; some gums genuinely need a visit every three or four months for a while, and your hygienist will say so rather than watch you lose ground between appointments. Equally, if your mouth is spotless, nobody here will book you in more often than you need.

Neither appointment is a test to pass, incidentally. The point of noting things from visit to visit is to steer the cleaning and the advice, not to grade your brushing, and the tone in the room reflects that.

What Airflow adds, and who should have it

Airflow is a £20 add-on to any hygiene appointment, and it does one job exceptionally well. A controlled jet of air, warm water and a very fine powder is swept across the teeth, dislodging the surface staining that tea, coffee, red wine and smoking leave behind, including in the awkward places a polishing cup cannot properly reach, such as between front teeth and along uneven edges. It is not whitening, and it does not change the underlying shade of the tooth; what it does is return teeth to their own natural colour by stripping away the film of stain sitting on top, which for regular tea and coffee drinkers is often a bigger visual change than they expect.

Patients tend to find it comfortable, and many actually prefer it to conventional polishing. Bleeding gums are a message, not a nuisance

Pink in the sink is so common that people file it under normal. It is not. Healthy gums do not bleed when you brush; bleeding is the earliest visible sign of gum inflammation, and gum disease at that early stage, gingivitis, is fully reversible with professional cleaning and better home care. Ignore the message for long enough and it can progress to periodontitis, where the bone supporting the teeth is gradually lost. That damage does not reverse, and gum disease is one of the main reasons adults lose teeth at all.

Pregnancy hormones can make gums bleed more readily, diabetes and gum disease each make the other harder to control, some medicines affect the gums, and smokers should know that smoking can mask bleeding while the disease advances quietly underneath. Bad breath belongs in this conversation too. Persistent bad breath very often traces back to the same bacteria that inflame gums, hiding in the same places brushing misses, and a proper professional clean tackles the cause rather than masking it with mints.
