Wisdom Tooth Infection: Symptoms, Treatment and When It Is Urgent

Wisdom teeth have a talent for terrible timing. They arrive in your late teens or twenties, often without enough room, and when the gum around a half-erupted one becomes infected the pain can go from a niggle to misery in a day or two. If your search brought you here at 2am with one side of your jaw throbbing, this covers what is happening, what will genuinely help before you can be seen, and the small number of signs that mean you should not wait at all.

The short answer

Infection around a wisdom tooth is usually pericoronitis: the gum flap over a partly erupted tooth traps food and bacteria, and the tissue becomes swollen, sore and sometimes infected. Mild cases settle with meticulous cleaning, warm salt water rinses and pain relief. Established infections need a dentist, who can clean under the flap properly and prescribe antibiotics where they are genuinely indicated. Repeated infections around the same tooth are usually the point at which removing it becomes the sensible fix.

The urgent exceptions: swelling spreading into your face or neck, difficulty swallowing or breathing, one eye starting to close, or a high fever with feeling genuinely unwell. Those are signs the infection is spreading, and they mean A&E or 111 now, not a dental appointment next week.

How to tell it is a wisdom tooth infection

Pain and swelling at the very back of the jaw, usually one side, often worse when biting because the opposite tooth presses on the swollen gum.

A bad taste or smell from the area, from trapped food and infected tissue draining.

Difficulty opening your mouth fully. Stiffness in the jaw muscles near an infected wisdom tooth is common and usually eases as the infection settles.

Swollen glands and feeling run down. Your body treats this like any other infection.

It is worth knowing what it might be instead. Pain further forward is more likely a decayed or cracked tooth, covered in our toothache guide, and a severe, throbbing pain with a swelling on the gum can be a dental abscess, which has its own rules. Jaw joint pain near the ear on opening and closing is usually the joint rather than a tooth.

What actually helps before you are seen

Clean the area, even though it hurts. This is the counterintuitive one. The infection is driven by trapped food and plaque under the gum flap, so gentle but thorough brushing around the tooth, and a soft interdental brush if you can reach, does more than anything else. Avoiding the area because it is sore feeds the problem.

Warm salt water rinses, several times a day, swished firmly around the back of the mouth.

Pain relief taken properly. Follow the packet, and ask your pharmacist what suits you. Do not hold aspirin against the gum, which burns the tissue.

Soft food, plenty of fluids, and skip smoking and alcohol while it settles, both slow healing.

What does not help: waiting for it to resolve permanently on its own. Mild pericoronitis often settles in a few days with good cleaning, but the gum flap and the trapped-food trap are still there, which is why it tends to come back.

What a dentist actually does, and what it costs

For the infection itself: the area is cleaned properly under the flap, which is uncomfortable for seconds and relieving within a day, sometimes with an antiseptic wash. Antibiotics are prescribed where there are signs of spreading infection or fever. They are not a routine fix, and used alone without cleaning they buy quiet weeks rather than a cure.

For the cause: if the same tooth flares repeatedly, removing it is usually the answer. Sometimes the opposite wisdom tooth is removed instead when it is the one biting the swollen gum. Frequently asked questions

Can a wisdom tooth infection go away on its own?

A mild flare of pericoronitis can settle within a few days with rigorous cleaning and salt water rinses. The underlying trap, a gum flap over a partly erupted tooth, remains, so repeat episodes are common. An established infection with spreading swelling or fever will not resolve on its own and needs treatment promptly.

Do I need antibiotics for a wisdom tooth infection?

Not always. The core treatment is cleaning the trapped debris from under the gum flap, which a dentist does far more effectively than you can at home. Antibiotics are added when there are signs of spreading infection, fever or facial swelling. Antibiotics alone, without the cleaning, usually mean the problem returns.

When is a wisdom tooth infection an emergency?

Go to A&E or call 111 immediately if swelling is spreading into your face or neck, you have difficulty swallowing or breathing, your eye is beginning to close, or you have a high fever and feel very unwell. Spreading dental infections can become dangerous quickly and are treated as genuine emergencies.

Should an infected wisdom tooth be removed?

After a single mild episode, not necessarily. After repeated infections around the same tooth, removal is usually the honest recommendation, because each flare tends to be worse than a planned extraction. An X-ray shows how the tooth and its roots sit, and whether removal is straightforward or better done by a specialist.
